Description of Dr. Fuster's presentation: The Keynote Lecturer, Valentin Fuster, addresses the primary public health challenge for the 21st Century: moving from a focus on the treatment of diabetes and heart disease, to protecting health through prevention. He shares a wealth of understanding from the current goals of the NIH, through describing the current understanding of the pathologies underlying atherosclerosis and diabetes, to treatment modalities. He discusses emerging therapeutics and diagnostics, including stem cells and multi-modality whole body imaging to measure burden of disease. He describes the interaction between non-modifiable risk factors and behavioural risk factors with disease states, and concludes with a presentation of his work with the World Heart Federation, Grenada Heart Project.

Description of Dr. Kahn's presentation: When considering obesity and its role in metabolic disease, it is clear that it is not simply the degree of adiposity that is important, but also the distribution of body fat. In this talk, Dr. Kahn suggests that increasing visceral adiposity is associated with decreasing insulin sensitivity independent of body size. Thus, even lean subjects may have increased visceral adiposity and insulin resistance and along with these, alterations in metabolic parameters that typify increased cardiovascular disease risk.

Description of Dr. Heagerty's presentation: The importance of small arteries in terms of maintaining constant blood flow to vital organs such as the brain, kidney and heart has been increasingly realized over the last ten years. In terms of high blood pressure it is clear that the most important change in small arteries is eutrophic inward remodelling. That is, there is a re-arrangement of pre-existing structural components in the vascular wall without evidence of hypertrophy. However, in patients with dyslipidaemia, there is evidence of endothelial dysfunction and of course patients with multiple risk factors, both structural changes and functional abnormalities of relaxation will co-exist. Heagerty presents new evidence that new classes of drug such as calcium channel blockers and antagonists of the renin angiotensin cascade can improve endothelial function and reverse remodelling.

Description of Dr. Krauss' presentation: The atherogenic dyslipidemia of the metabolic syndrome consists of interrelated increases in plasma levels of triglyceride-rich lipoproteins (TRL) and small LDL particles, and reduced HDL. According to ATP III, atherogenic dyslipidemia can become a target for lipid-lowering therapy after the goal for LDL-C has been attained by therapeutic lifestyle change with the addition of statin or other cholesterol-lowering drugs as needed. Additional lipid treatment options currently available include nicotinic acid, fibrates, and omega-3 fatty acids, and other modalities are in development. In this lecture, Dr. Krauss summarizes the current pharmacologic approaches used to ameliorate the atherogenic dyslipidemia of metabolic syndrome.
