CMSC 2008 Annual Meeting

A Certificate of Attendance will be given upon completion of course requirements enabling you to register your credit with the appropriate licensing boards or associations. You may apply for other accreditations using the procedure established by the specific organization. Additional brochures and evaluation forms will be provided upon request. Physicians, Nurses, Pharmacists and Social Workers will receive a certificate with the appropriate number of credit hours.

 

Physicians: This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of BioSymposia, Inc. and the Consortium of Multiple Sclerosis Centers. BioSymposia, Inc. is accredited by the ACCME to provide continuing medical education for physicians.

 

BioSymposia, Inc. designates this educational activity for a maximum of 23.75 AMA PRA Category 1 CreditsTM. Physicians should only claim credit commensurate with the extent of their participation in the activity.

 

Nurses: This offering of up to 23.75 contact hours is provided by BioSymposia, Inc., which is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation. Successful completion of this program includes filling out an evaluation form. Statements of credit will be mailed to all participants within four weeks of the program date. BioSymposia, Inc. is an approved provider by the California Board of Registered Nursing, Provider Number CEP 8210, an accredited approver by the American Nurses Credentialing Center's Commission on Accreditation, for 23.75 contact hours.

 

The American Nurses Association's accreditation refers to recognition of educational activities only and does not imply approval or endorsement of any product of any company displayed in any form during the presentation of educational activities.

 

Social Worker: This program has been approved by the National Association of Social Workers (provider #886419450) for 23.75 continuing education contact hours.

 

Pharmacists: This program provides 23.75 contact hours of continuing education credit (2.375 CEUs) for pharmacists. Successful completion of this program includes filling out an evaluation form. Statements of credit will be mailed to all participants within four weeks of the program date.

 

BioSymposia, Inc., is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. The program identification number is 340-999-08-001-L01-P.

 

Continuing education credit is not offered during meals (except for the presentation during the opening ceremony and the Meet The Professor Dinner - Clinical Controversies), breaks, exhibits, receptions/cocktail party and any private group meeting (e.g., committees, research interest groups, business meetings).
